Chargebacks are the largest gross-to-net deduction in US pharma. A wholesaler buys at WAC and sells to a contract pharmacy or hospital at a lower contract price — the manufacturer reimburses the difference. Chargebacks are estimated based on historical chargeback rates by customer class and provisioned at the time of sale to the wholesaler.

The system applies a chargeback rate (estimated percentage of gross revenue) based on historical settlement data by product and customer class (340B, GPO, VA, retail pharmacy). The provision is posted to the GL as a contra-revenue deduction and a chargeback liability. Actual chargebacks submitted by wholesalers are processed against the provision — shortfalls or excesses create chargeback adjustment entries. The provision balance represents the estimated amount still to be submitted.

Chargebacks are one of the highest-risk estimates in pharma accounting. Auditors test the provision rate assumptions against historical actual chargeback data (rolling 12-month actual-to-estimate ratios). Test that 340B eligible pharmacy volumes are correctly captured in the higher-rate chargeback class. Reconcile the chargeback provision balance to the outstanding estimated claims pipeline from the RDM system. For drugs with significant specialty or government channel mix, test whether the channel mix assumption is current. Review for any changes in contract pricing that would shift the chargeback rate.
