Reimbursement claims are submitted by the insured via a claim form (paper or online portal) with original invoices attached. The claims handler validates the eligibility (is the treatment covered, is the insured within the annual limit?), applies the co-payment and deductible, and approves the net reimbursement amount. The payment is made directly to the insured's bank account via the AP module. Original invoices must be retained for audit purposes — they should not be returned to the insured.

⚠️ Audit Flags

Auditors test that original invoices are held on file (not copies), as reimbursement fraud often involves submitting invoices multiple times or to multiple insurers. Verify that the annual benefit limit tracking is accurate — overreimbursement beyond the limit is a claims leakage issue. Test that the co-payment and deductible are correctly deducted from every reimbursement. Confirm that payments are made to the insured (not a third party) unless a signed assignment has been obtained.
