Direct billing networks allow insured patients to receive treatment without upfront payment. The hospital bills the insurer directly. Claims are expensed when the service is rendered, based on pre-authorization approvals.

Direct billing claims flow from the TPA's pre-authorisation system to the insurer's claims management system upon discharge. The hospital submits the final invoice electronically via the TPA portal. The claims/TPA system validates the invoice against the pre-authorisation: approved diagnosis, treatment codes, and amounts. Validated invoices are posted to AP as payable to the network hospital. Invoices with discrepancies are flagged for manual review. Batch payment to hospitals is processed monthly or bi-monthly.

⚠️ Audit Flags

Auditors test the control over pre-authorisation vs. final invoice matching — a material proportion of invoices exceeding the pre-authorisation amount without approval is a control weakness. Test a sample of claims for eligibility (is the member listed on the policy, was the treatment within the benefit limits?). Review the hospital network agreement for the agreed fee schedule — invoices above the agreed rates should be rejected or renegotiated. Also check for duplicate claims submissions.
