💡 Accountant's Note

When a hospital admission is pre-authorized, the insurer has a present obligation even before the invoice arrives. Accruing approved but unbilled claims improves IBNR accuracy and ensures liabilities are not understated.

The TPA system maintains a real-time register of all open pre-authorisations (approved but not yet invoiced). At month-end, this register is extracted and used to calculate the pre-authorisation liability — essentially unbilled claims where the obligation exists. The accrual is posted as a top-side journal to the GL and reverses in the following month when the actual invoices are received and posted. This accrual is a key component of the medical IBNR estimation process.

⚠️ Audit Flags

Auditors obtain the open pre-authorisation register directly from the TPA and independently verify that all approved admissions above a threshold are included in the accrual. The risk of understatement is significant if pre-authorisations are not systematically tracked. Test that the accrual reverses correctly in the following period and that actual invoices match or exceed the accrued amount. Assess whether the pre-authorisation accrual is adequately supplemented by additional IBNR for claims not yet pre-authorised.
